Supported Group Living

Supported Independent Living & Transitional Aged Youth

Individualized Supports

Supported Group Living

Offered in homes across Southern York Region, the Supported Group Living program provides 24/7 support. Our well-maintained single-family homes support 4-6 individuals each, providing a full range of support to help people achieve their goals and live a happy and vibrant life. 

What it provides

Our specially trained teams support adults with a variety of different needs including but not limited to aging, medical complexities and challenging behaviours.

 

Supports may include activities of daily living such as assisting a person with personal hygiene (dressing, grooming) meal preparation, administration of medication, and include life skills such as money management, banking, using public transportation, connecting to their community and goal planning.  

 

Our teams work in partnership with other health and social care providers in the community to promote inclusion, individual choice and independence.

Eligibility

Supports are provided to individuals who are at least 18 years of age, and who have a developmental/intellectual disability.

Eligibility

Supports are provided to individuals who are at least 18 years of age, who have a developmental/intellectual disability and who are living in their own home in the community. 

Supported Independent Living (SIL) &
Transitional Aged Youth (TAY)

Individualized Supports that are tailored to meet the needs of the person supported.  SIL supports are up to 6 hours of support a week. TAY Supports are individualized based on predetermined needs.  

What it provides

SIL supports activities of daily living such as assisting a person with personal hygiene, dressing, grooming, meal preparation, administration of medication, support for money management, banking, using public transportation, connecting to services in the community, being a good tenant, education, employment and other life skills.

TAY level and duration of support are determined through each youth’s individualized transition plan, ensuring services are tailored to their unique strengths, needs, and goals. The program provides services including life-skills training, housing support, education and employment assistance, and emotional and mental health resources.

Individualized Supports 

Provides a range of supported options for individuals and their families with individualized funding who are seeking customized services that best meet their needs and goals while promoting growth and independence.  

What it provides

Service contracts may include full-time or part-time out-of-home placements, in-home supports, behavior management, and other services tailored to the needs of the individual and their family.

 

Services may also include supports that help families care for their family member at home.
