Community Support Program
Guided by its motto to help individuals and families help themselves, the Community Support program offers holistic, individualized, and multilingual services to people of all ages with intellectual disabilities in Southern York Region.
This service provides personalized supports and guidance to individuals and their families, ensuring timely access to services with no waitlist.
What it provides
The Community Support Program serves over 2,000 individuals and their families through education, system navigation assistance, and access to community resources. The program promotes self-sufficiency, expands opportunities, and fosters strong community partnerships to enhance overall well-being. It supports individuals to make informed decisions, develop independence, and participate fully and inclusively in their communities.
This service also offers educational and wellbeing workshops for people supported and their families, respite opportunities including summer camps and specialized programming, family-led initiatives, and collaborative community partnerships that promote inclusive and supportive communities.
Eligibility

A person must have a confirmed diagnosis of intellectual disability. (Documentation of diagnosis by a certified psychologist/medical professional may be requested) .

Reside in Southern York Region (Markham, Richmond Hill, Vaughan and/or Stouffville) .

There is no age limit for referral to the Community Support Program.
