Achieving Community Transitions (ACT)
The ACT Initiative is dedicated to supporting individuals with intellectual disabilities, ages 15–50, across York Region in achieving their goals related to employment and/or independence.
By focusing on each person’s strengths, we provide individualized support, job readiness and retention, skills development, and strong community connections. Through this approach, individuals gain the tools to reach their career goals, build confidence, and create sustainable opportunities.
What it provides
